• One-on-one therapy sessions
• Home exercise programs
• Progress monitoring and adjustments

3. Patient Responsibilities

As a patient, you agree to:

• Provide accurate and complete medical information
• Follow treatment plans and home exercise programs
• Attend scheduled appointments or cancel with appropriate notice
• Pay for services rendered in accordance with our payment policy
• Communicate any concerns or changes in your condition
• Respect our staff, facility, and other patients

4. Appointment and Cancellation Policy

Scheduling: Appointments can be scheduled by phone, online, or in person. We will make every effort to accommodate your preferred times.

Cancellations: We require at least 24 hours notice for cancellations or rescheduling. Late cancellations or no-shows may result in a fee or affect your ability to schedule future appointments.

Late Arrivals: Please arrive 10 minutes early for your first appointment. Arriving late may result in a shortened session or rescheduling.

5. Payment Terms

Payment is due at the time of service unless other arrangements have been made. We accept:

• Cash, check, and major credit cards
• Insurance (we will bill your insurance as a courtesy)
• Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA)

You are responsible for any amounts not covered by insurance, including deductibles, co-payments, and non-covered services.

6. Limitation of Liability

Health 1st, Inc. and its employees shall not be liable for any indirect, incidental, special, consequential, or punitive damages resulting from your use of our services. Our liability is limited to the amount you paid for services.

7. Consent to Treatment

By using our services, you consent to physical therapy treatment and related services. You acknowledge that physical therapy involves physical contact and exercise, which may result in temporary soreness or discomfort. You will be informed of treatment risks and benefits before beginning care.

8. Modifications to Terms

We reserve the right to modify these terms at any time. Changes will be effective immediately upon posting to our website. Your continued use of our services after changes are posted constitutes acceptance of the modified terms.
